Credit Recovery Application Form
ONLINE APPLICATION
Complete this online application to enrol in American High School Academy's High Credit Recovery program.
If you are applying for Credit Recovery without the help of your current school counselor you must print and complete a Credit Recovery Authorization Form.
This printed and completed form MUST be signed by your school's authorized representative and uploaded when you submit your online application for us to enrol you in the Credit Recovery program.
Please do not begin your online application until you have a digital copy of the signed form ready to upload as, for privacy reasons, you cannot save this form.
If you are applying for Credit Recovery with the help of your current school, then you only have to provide your school and counselor's name when completing the form.
Registration fee and tuition payment can be made with credit card or PayPal.
PROGRAM FORMAT
Default registration is for ONLINE study, with all course materials, assignments, and quizzes delivered and completed online.
If you don't have access to a computer, you can indicate that you need PRINTED course materials, assignments, and quizzes and these will be provided when your enrollment is confirmed. The cost for printed materials is $30 per course.
IN-PERSON APPLICATION
To apply for credit recovery enrollment with check or cash payment please book an appointment with an Admissions Advisor to complete your application in person. If you are applying without the help of your current school, please bring the completed and signed Credit Recovery Authorization Form with you to your appointment.ADMISSION REQUIREMENTS
To process your application and confirm, enrollment in the credit recovery program, the American High School Academy Admissions Office must receive the following:
- Completed and Signed Credit Recovery Form, if applicable OR the name of your current school and School Counselor;
- Your student ID number
- Tuition fees.
IMPORTANT
Please have ready a digital copy of a valid government issued ID for the student (and parent/guardian if the student is under 18 years old) as ID will be required to submit this form.
Acceptable file formats are .jpeg, .pdf, .gif or .png only. Maximum file size is 8MB.

Most students are referred to American High School Academy by their school districts. However, students and family members can contact us directly to learn more or to enroll in our programs.
